Treatment of androgenetic hair loss for men

How do we treat male androgenetic hair loss?

At Maritime Hair Center, the treatment of androgenetic alopecia is based on personalized combination therapies that effectively inhibit the progression of hair follicle miniaturization and stimulate new hair growth. We implement this technical process through:

  • Pharmacological treatment: The use of 5-alpha reductase inhibitors (Finasteride, Dutasteride) alongside drugs that activate miniaturized or dormant hair follicles (Minoxidil). Pharmacotherapy is selected either in oral form or as topically applied, custom-formulated compound scalp lotions.
  • Complementary treatments (clinical procedures): Intensive stimulation of the scalp to maximize therapeutic results and, in advanced stages, FUE hair transplantation.

Why do men lose hair?

The primary cause of male pattern androgenetic alopecia is the hypersensitivity of hair follicles to DHT (dihydrotestosterone), increased levels of this hormone, and elevated levels of free testosterone (unbound to SHBG protein).

It is worth noting that these factors do not have to occur simultaneously – very often, blood hormone levels appear completely normal, yet advanced miniaturization of follicles still occurs due to their genetic hypersensitivity.

Mechanism of action: Testosterone circulating in the body is converted into its active form – DHT – by the 5-alpha reductase enzyme. This hormone binds to receptors within the hair follicles, leading to their gradual weakening, shrinkage, and ultimate loss, which the patient perceives as hair thinning. The highest concentration of DHT-sensitive follicles is located in the temples, hairline, tonsure, and crown areas. The occipital and temporal regions of the scalp are typically free from this sensitivity.

Main medications used in therapy

The baseline for halting hair loss is blocking the 5-alpha reductase enzyme, which reduces the amount of destructive DHT circulating in the blood and protects hair follicles from damage.

1. Finasteride

Type II 5-alpha reductase inhibitor

Lowers blood DHT levels by approximately 60%. It breaks down in the body with a half-life of about 5 hours, while the hormone-lowering effect is maintained for up to 24 hours. Because of this, the medication must be taken consistently every day at the same time.

2. Dutasteride

Type I and II 5-alpha reductase inhibitor

Acts more potently than finasteride, reducing blood DHT levels by up to approximately 90%. Its terminal elimination half-life in the body is about 5 weeks, and the therapeutic effect can persist for up to 6 months. It is most commonly administered in oral form (daily).

Possible side effects of pharmacotherapy

As with any pharmacological treatment, blocking 5-alpha reductase carries a risk of adverse effects. The most frequently reported include:

Erectile dysfunction and decreased libido
Ejaculatory disorders
Mood drops / mood changes
Dizziness

Minoxidil – growth activator

Another crucial pillar of treatment is Minoxidil, which we apply as a topical lotion, orally, or by combining both forms to achieve maximum results. This drug:

  • Increases blood flow to the scalp and thickens the hair shaft.
  • Prolongs the active growth phase of the hair (anagen).
  • Stimulates dormant hair follicles (those in the telogen phase) back into growth.

Hair loss treatments without side effects?

While the risk can never be fully eliminated, modern trichology allows us to reduce systemic side effects to a minimum or eliminate them entirely.
